The sequel to Trier's Europa trilogy. The Element of Crime (1984) and Europa (1985) are the other two films in the trilogy (1991). The film is about the screenwriting process and was co-written by Niels Vrsel. Vrsel and von Trier play themselves as they scramble to write a script for a producer at the last minute. This story is intercut with scenes from the film they are writing, in which von Trier plays a renegade doctor attempting to cure a modern-day epidemic.
