Officer Daniel Carter comes across a blood-soaked man staggering down a remote stretch of road while on routine patrol. He transports the young guy to a nearby rural hospital with a skeleton team, only to find that the patients and staff are mutating into something horrible. As the terror grows, Carter leads the other survivors on a perilous journey into the hospital's basement depths in a last-ditch effort to put a stop to the nightmare before it's too late.
