Caine, the dentist from the original film, has escaped from the psychiatric facility where he has resided since he was captured. It isn't easy for him to get his life back on track, but he makes his way to a small Midwestern town and pretends to be the town dentist. Caine's life begins to improve until the day he sees his new love being held by someone else. After this, he goes on another killing spree, this time targeting his unsuspecting patients, as in the first movie.
