Nica Pierce has been held captive in a facility for the criminally ill for the past four years, and she has come to the erroneous conclusion that she was the one who was responsible for the deaths of her entire family, and not Chucky. But when her psychiatrist introduces a new therapeutic "tool" to facilitate his patients' group sessions— an all-too-familiar "Good Guy" doll with an innocently smiling face— a string of grisly deaths begins to plague the asylum, and Nica begins to wonder if maybe she isn't crazy after all. The "Good Guy" doll is a familiar doll with a face that smiles innocently. In the meantime, Chucky's adversary from the first three Child's Play films, Andy Barclay, who is now an adult, rushes to Nica's assistance. However, in order to save her, he will need to go through Tiffany, Chucky's ex-wife from a long time ago. She is willing to do anything, no matter how dangerous or vile, in order to assist her beloved evil devilish doll.
